Outreach and Navigation

Empowering People’s Independence and the Urban League of Rochester joint Outreach Services provide education, navigation, linkage and follow along support for individuals with intellectual and/ or developmental disabilities and their families in Monroe County. We can help to break down barriers, educate and empower, navigate services and offer 12 months of follow along support after OPWDD linkage. Our approach is to work to meet individuals and families where they are at (in person, virtually, in community or at home), help provide linkage and referrals, and provide education and support. This program is funded by OPWDD Family Support Grant. Contact us via:
